- Duties
- Contractor shall provide outpatient pharmacy services to patients ranging from neonate to geriatric.
- Contractor will perform work in the following work areas as directed, screening patient electronic health record for appropriateness of the medication order or refill, verifying patient prescriptions are accurately filled, instructing for use are clear, counseling patients to ensure proper use of the medication.
- Contractor will perform his/her duties in the outpatient pharmacy department.
- Contract pharmacist order screening duties will include a thorough review of the patient’s electronic health record to verify the appropriateness of therapy. The following screening duties list is not exhaustive, but rather constitutes the minimum expectation: appropriate dose for the condition, weight, age, and renal/hepatic function of the individual patient, appropriate medication for condition, monitoring for drug-drug or drug-disease interactions, and verification that day supply matches what is ordered.
