Job Summary: Under general direction collects, analyzes data, identifying expected outcomes of each patient, developing a plan of care with interventions and constantly evaluating the patient's progress toward attainment of the outcomes, working collaboratively with the specialty healthcare team and other disciplines.
Responsibilities:
- Apply critical thinking and problem-solving skills in a dynamic, systematic, and collaborative approach to providing and coordinating health care services for a defined population.
- Use advanced nursing specialty knowledge to assess, plan, implement, evaluate, and interact with patients.
- Evaluate options and services to meet individual health needs through effective communication and available resources to promote quality, cost-effective outcomes.
- Develop and maintain patient tracking and reporting programs to identify gaps in the health care delivery system.
- Participate in organizational activities such as unit-based councils and process improvement committees to continually evaluate and improve services for the specific population.
- Independently and consistently apply nursing knowledge to collect and analyze patient health data, determine diagnoses, identify individualized expected outcomes, develop and implement care plans, and evaluate patient progress in collaboration with the healthcare team.
- Engage in systematic evaluation of the quality and effectiveness of nursing practice, including research and quality improvement initiatives to drive positive changes.
- Evaluate personal nursing practice against professional standards and regulations and set short- and long-term professional goals.
- Adhere to the Standards of Practice for Case Management and the ANA Nurses Code of Ethics and apply relevant specialty standards of practice.
- Contribute to the professional development of peers and others within a Shared Governance model of nursing practice.
- Act as a consultant on specialty issues across disciplines.
- Make ethical decisions and act on behalf of patients, delivering care in a nonjudgmental, nondiscriminatory manner that respects patient diversity, autonomy, dignity, and rights.
- Collaborate with patients, families, and healthcare providers to establish care goals and plans, and make decisions related to care and service delivery.
- Consider safety, effectiveness, evidence-based practice, and cost in planning and delivering patient care.
- Assist patients and families in identifying and securing appropriate services to meet health-related needs, including coordination with various insurance payers.
- Plan, collaborate, and coordinate with external facilities to arrange care for complex specialty needs not available locally.
- Utilize advanced problem-solving and critical thinking skills to triage patients via telephone, email, and walk-in interactions.
- Provide extensive patient education on complex specialty issues and multiple disease processes.
- Deliver timely and effective nursing care according to established standards, addressing physical, social, and emotional needs.
- Demonstrate flexibility and adaptability in response to new and changing situations.
