Specialty: SDPI (Special Diabetes Program for Indians)
Work Setting: Clinic – may also assist with mobile unit and assist with in-home visits
Schedule: Monday – Friday, 40 hours per week
Facility Type: Critical access hospital
Schedule: 5×8 hours, open to 4×10 hours as well
Credentialing Timeframe: 2 weeks
Requirements:
Licensure: unrestricted RN license from AZ or other compact state OR Registered Dietitian (RD) or Registered Dietician Nutritionist (RDN).
Education: Bachelor's of Nursing (BSN)
Experience required: 2 years of diabetes care and 2 years providing direct care to patients; preferring 2 years of case management and/or care coordination.
Certifications: BLS, ACLS, strongly preferring Certified Diabetes Care and Education Specialist (CDCES) credential
